Various upload buttons merge to Upload Media

This has come a long way, see [2134]. It's a 1.5 priority but never had a ticket for some reason.

Still TODO:

Fix it so when I add some bogus things and some good things I don't get a validation error on the multiple upload page. All we need to know is that at least one file was uploaded, then we're ready to go to the annotation page directly.

Support Flash/HTML5 multiple file upload as an option. Make sure that can handle more than 6 (?).

Open a ticket to look at embed media too (right now it's just the old embed video workflow).

I fixed the validation problems. All validation now happens on the annotation page except for a check to ensure you have at least one file: [2136] This simplifies the code and gives us less to style on the initial upload form.

Looks like "batch upload" (meaning multiple select in one browse button) has been pushed off to 2.0, so this ticket is done. I didn't do anything exciting with Embed Media, but the video embed workflow is pretty reasonable. We should talk about whether there's time to do anything more there.

I opened a related ticket for Jake to make a bunch of icons, that does need to get done before this looks nice.
