Media plugin should support dumb PDF handling when ghostscript is unavailable

Description

Currently the media plugin produces errors if you try to use PDFs without ghostscript. This should be changed so that a static PDF icon is displayed in place of a nice snapshot when ghostscript is not available.

This is fixed, it displays a PDF icon of a fixed size in a white box of the requested size.

John, please test this for aesthetic acceptability. Shut off netpbm in a project if you haven't already got one where it's shut off:

Then upload a PDF and see what you think of the way that plays out both in the media repository and when actually inserted on a page. If there are actual errors assign back to me, if there are aesthetic concerns fix it if you can or rope me in.

(Don't leave netpbm shut off in a commit of asandbox or demo, please. Fine to tweak them and put them back.)
